Liability, Medical and
Marketing Release with Equipment Replacement Acknowledgement
I hereby release John Payette
and any of his sponsors, of any and all liability in the event of an accident
or injury related to my fishing trip or demo boat ride or activities with John
Payette (including but not limited to trip prep, travel to and from the fishing
site or demo site, boating related to the fishing trip, and any post-fishing
trip or demo ride activities). I also consent to any emergency medical and/or
hospital service that may be deemed necessary and be rendered at designated
hospitals, by a duly licensed physician in the event such need arises. I also
grant permission for John Payette to use on his website or other advertising
avenue, any likenesses of myself or my equipment (including "catches"
of fish or other life forms) taken during my fishing trip or demo ride with
John Payette (including but not limited to trip prep, travel to and from the
fishing site or demo site, boating related to the fishing trip, and any
post-fishing trip activities or demo ride).
In addition, I understand that
during the course of any trip or ride with John Payette, various equipment may
become damaged, lost or destroyed, including but not limited to such items as
rods, reels, life vests and similar fishing or non-fishing equipment. I further
understand that if I am in part or in whole responsible for such damage, loss
or destruction of equipment, I will be responsible for reimbursing John Payette
for the damage, loss or destruction.
